I've run the Crew Management scenario a few times, and I've made it through with no malfunctions, but I still lose big-time due to morale.

If you are using your own design with a Galley in it, I'm not sure what to say, I've run into the same issue, but I've always thought that it was because the ship did not have any galley or anything like that.
